Not overwhelmingly smoky, but enough to let you know there's peat. Give it some time on the tongue, and the sweet malty backbone shines through. Reminds me of a big, aged, malty beer -- like barleywine, especially if a more English bent-- with more of everything. Smoke doesn't seem medicinal, but adds a nice bite.
